Don't forget quality

As certified Quality Auditors, we know how to recognize compromises in the integrity of your notes.  When performing audits, we see over and again compromises of quality in exchange for technology.  EMR vendors sell the idea of eliminating transcription as the way to cut costs, yet they don't know national charting standards and how to implement these into their point-and-click protocols.  Physicians are often forced to change the way they see patients to fit the software.  Providers take on additional risks when patient findings fall outside the "pick lists", and finished notes fall short of expected quality and do not accurately represent the actual exam.  Even today, "Not charted, not done" still holds true.
